Dr. David Hanekom, MBBCH (Rand), F.A.C.P., C.M.P.E., and Chief Medical Officer at SPH Analytics has been selected to serve on the Board of Directors for the Consortium of Southeastern Hypertension Control for a 3-year period beginning January 2016.
Dr. Hanekom is committed to improving outcomes for patients with chronic cardiovascular conditions. Hanekom guides development of population health products for SPH Analytics (SPHA). Under his direction, SPHA products identify care opportunities that allow clinicians to prioritize patients based on chronic care needs including hypertension. His appointment to the Consortium of Southeastern Hypertension Control™ (COSEHC) Board of Directors will allow him to use his expertise to meet the goals of COSEHC as they relate to preventing and treating cardiovascular disease and strokes.
Dr. Hanekom obtained his medical degree from the University of the Witwatersrand Faculty of Health Sciences in Johannesburg, South Africa, and has completed postgraduate training in family medicine at the University of Limpopo School of Medicine, Pretoria, South Africa, and internal medicine at the University of North Dakota School of Medicine and Health Sciences, Grand Forks, North Dakota. He is a Fellow of the American College of Physicians and previously a Clinical Associate Professor of Medicine at the UND School of Medicine and Health Sciences. As a previous Chief Medical Officer at a nationally branded health plan and Director of Chronic Disease Management at Sanford Health System North, Dr. Hanekom has deep expertise in population health management and strategies which improve healthcare outcomes.
About the Consortium of Southeastern Hypertension Control (COSEHC)
COSEHC is a nonprofit (501c3) organization established to improve cardiovascular outcomes in the Southeastern United States, which has the highest prevalence of hypertension-related morbidity and mortality. COSEHC promotes scientifically based research and education activities to reduce vascular disease and its complications. The organization encourages physician participation in its clinical database that allows for on-line consultation of members and coordinates and facilitates evidence-based best practices for participating network physicians. COSEHC also provides ongoing educational opportunities to current practitioners through continuing medical education (CME) credits and an academic-clinical partnership to promote quality and outcome improvement in the region.
